Where can we go to get cures for the following conditions: poison, disease, curse, level drain, lycanthropy, polymorph, alignment change, death, undeath?

Phoebus and Nyxa can handle poison, disease, curses, lycanthropy, and death as well as sleep, barbs, rot grubs, petrification, and possession. They can restore crippled limbs and eyes to normal and even regrow lost limbs and eyes (for an additional fee, we can expedite the regrowth process!).

DFRPG does not have any polymorph spells, so reversing that would require a trip to at least Greyport if not further afield. Unless, of course, Rajjij knows something about shapechanging. GURPS does not have alignment but personality change is likely reversible with remove curse. Restoring undeath can probably be accomplished via resurrection.
